A Proactive and Supportive Self-management Health Program

Viverae’s Chronic Condition Management (CCM) program targets the top five chronic conditions that typically have the highest impact on an organization’s health-related costs.

  • Asthma
  • Chronic Obstructive Pulmonary Disease
  • Congestive Heart Failure
  • Coronary Artery Disease
  • Diabetes (types I and II)

Program goals

Viverae’s goals for CCM look to address and improve the overall health of company employees. Specifically, the Viverae team looks to:

  • Support treatment adherence
  • Support member daily self management
  • Encourage and support healthy behavior choices
  • Reduce disease-related complications
  • Educate members on disease condition(s) to enhance self management of the disease(s)
  • Identify barriers that impact a member’s ability to improve health outcomes
  • Track outcomes to identify opportunities to improve the health management program

Program purpose

Viverae’s CCM program supports each member’s specific medical treatment plan. Based on nationally recognized clinical guidelines and standards, the program incorporates the following outreach enhancements:

  • Identification of members with certain chronic conditions
  • Outreach and encouragement to become engaged in healthy lifestyle behaviors
  • Comprehensive questionnaires and assessments
  • Stratification capabilities of risk factors
  • Specific interventions for each condition
  • Educational materials specific to each condition
  • Creation of individual care plans
  • Tracking of outcomes
  • Disease and wellness activities designed per company specifications

In addition, members in CCM also have access to Viverae’s Health Management System (VHMS), the online portal for individuals to view progress toward goals and track all wellness-related activities.
