Member Rights and Responsibilities

Members who participate in the Viverae Chronic Condition Management Program (the “Program”) have certain rights associated with the services they receive as well as certain responsibilities for cooperating with those that are helping them with their chronic conditions.

Member Rights:

  • You have the right to have access to information about the Program and other Viverae services.
  • Your individually identifiable medical information (“Protected Health Information” or “PHI”) will be kept confidential in accordance with The Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).
  • You have the right to know who has access to your PHI in the context of the Program and the procedures in place to keep your PHI confidential.
  • You have the right to be treated with the utmost courtesy and respect.
  • You have the right to know which member of the Viverae staff is responsible for helping you manage your condition and how to request a change if you so desire.
  • You have the right to access the current clinical practice guidelines utilized in the Program, and you are encouraged to discuss all the guidelines and all treatment options with your physician.
  • You have a right to utilize Viverae staff as patient advocates on your behalf.
  • You have the right to opt out of the Program at any time. Your participation in the Program is voluntary; however, opting out may impact your employer’s incentive qualification requirements.

Member Responsibilities:

  • You are expected to provide Viverae with accurate contact information and as much clinical information as possible to the Viverae professional staff in order to assist you in managing your chronic condition.
  • You are expected to work with a primary healthcare provider that helps coordinate your care, and to provide Viverae with their name and contact information
  • You are expected to actively participate in the Viverae chronic condition management program by following Viverae recommendations, treatment(s) prescribed and recommendations by your physicians, reading educational materials, knowing personal health goals and following up on action items to track your progress.

The Viverae Chronic Condition Management Program (“Program”) is designed solely to assist you and your physician in understanding the health impact of your lifestyle and behavioral choices, to support you in making choices that reduce your health risks and improve your overall quality of life, and to encourage you to learn about, understand, and follow the specific treatments recommended by your physician. The Program is not intended to be a substitute for professional medical or other advice, for which you must consult your physician or other health care practitioner. Only your physician or other health care practitioner is qualified to provide professional health care advice, diagnosis or treatment, and you should never delay or avoid seeking such advice or treatment based on any materials, discussions, or communications provided through or arising from the Program.

Viverae develops and manages chronic condition management programs solely for the benefit of the members of its employer clients. Our programs are not supported with outside funds, although, in some cases, Viverae has received permission to use member educational materials or other in-kind member-oriented products that have been provided by governmental and non-profit organizations.
