Provider Rights and Opportunities

Viverae believes that all parties—employers, employees, and providers benefit when a workforce reaches its optimum level of wellness, self-management and functional capability. Viverae values the importance of the physician-patient relationship and respects the rights of physicians. The program is based on open and collaborative communication between you, your patients, and Viverae staff. The Viverae Chronic Condition Management Program (“Program”) is a voluntary program for our eligible members and is provided at no cost to them. When a patient agrees to participate in the Program, that entitles you to certain rights and opportunities as provided below.

Provider Rights:

• You have the right to receive written information about the Program.
• You have the right to be informed of the member’s action items and interventions provided by the Viverae clinical team.
• You have the right to contact Viverae for any questions or concerns.
• You have the right to be treated with the utmost courtesy and respect.
• You have the right to speak to the Viverae clinical staff as well as the Viverae Medical Director for any concerns regarding the program.

Provider Opportunities:

• You have the opportunity to support your patients’ health improvement by providing information to Viverae’s professional staff to reinforce your prescribed treatments as the patient also works with us in achieving the best clinical result possible.
• You have the opportunity to expand patients’ awareness as you listen to member concerns and provide professional feedback to the Viverae staff.
• You have the opportunity to be supported by Viverae in educating the member on nationally recognized standards of care to ensure they are informed about how to effectively manage their condition.
• You have the opportunity to encourage preventive measures and positive lifestyle behavioral changes to assist with managing their condition.

Our members and their medical providers decide what medical care members receive and how they receive it. It is not the intention of Viverae to provide specific medical advice, but rather to provide members with information to better understand their overall health. Viverae urges members to consult with appropriate physicians for diagnosis and answers to their personal health questions.

Viverae develops and manages chronic condition management programs solely for the benefit of the members of its employer clients. Our programs are not supported with outside funds, although, in some cases, Viverae has received permission to use member educational materials or other in-kind member-oriented products that have been provided by governmental and non-profit organizations.
