Assess
To establish the baseline risk factor profile of the employee population, Viverae performs a comprehensive assessment procedure using:
- Member Health Assessments (MHA) to provide self-reported lifestyle and behavioral information.
- Biometric Screenings to provide current, validated health information such as cholesterol, glucose, weight, body fat percentage, and blood pressure.
- Claims Analysis Capabilities and Clinical Compliance Coaching to map important medical quality and risk measures to the levels of clinical compliance within the group. The ability to identify “failures of care” (clinical non-compliance), supports enhanced coaching protocols to increase employees’ awareness of the issues to discussed with their physicians. This process helps increase individuals’ accountability to their treatment plans and ensures that our coaching efforts are supportive of their clinical needs. (Viverae is one of the few wellness companies to offer these increasingly important tools.)
PROGRAM DESIGN
By gathering health metrics through MHAs, Biometric Screenings and Claims Analysis, we design a custom program and Cultural Action Plan. Behavioral change support is provided to employees in these areas:
- Nutrition and lifestyle habits
- Physical activity levels
- Stress coping skills
CULTURAL ACTION PLAN
We provide cultural support to employers to facilitate work, life, and family balance. As employees go through the process of personal change, the organization is going through a parallel process of cultural change that fully supports the individuals’ efforts. A Cultural Action Plan includes the following:
- Program launch
- Incentives planning and tracking
- Wellness team training
- Activity development
- Program branding
- Program marketing support
